生老病死と医療の収斂

【プロフィール】1953年宇都宮市生まれ。獨協医科大学卒業、医学博士。同大第一内科講師後、栃木県で開業。現在藤沼医院院長、獨協医科大学心臓・血管内科非常勤講師、栃木県警察医。自らの経験に基づき、病気を悪と捉えずその存在意義を探求し、人間を霊・心・体と捉え三位一体の全人医療の実現をめざしている。

前回、永田勝太郎博士の会頭講演では「人間の実存的な痛み」における考察がなされました。今回は人として免れることのできない根本的な四つの苦しみ、すなわち生まれること、老いること、病気をすること、死ぬことの四苦と言われる生老病死とそれに対峙する医療との収斂について考えてみたいと思います。
この世に生を享けるにはまず母子共に生みの苦しみを乗り越えなくてはなりませんが、超未熟児でも出産が可能になった反面、回復の見込みのない延命治療などは、それをどこまで続けるかが医療者、家族を悩ませる結果となり、重症新生児の安楽死（フローニンゲン・プロトコール）という人の生死を人間がコントロールせざるをえないジレンマを生み出しています。
遺伝子医学は将来、老化をもコントロールし、超長寿命の実現が可能となるかもしれません。しかし人の幸福は単に長生きをするだけでは得られるものではなく、その人の人生への考え方や生き方によるところが大といえるでしょう（ブータン国の幸福度指数など）。医科学の進歩による超高齢化社会が生み出すであろう様々な歪みと葛藤を未来社会は受け止め、収斂することが求められます。
病を無くすことは医学の永遠の課題であり使命であります。しかし現実は医学の進歩にも拘わらず、過去にはなかった原因不明の難病が次々に生まれているのも現実です。又医学により沢山の病気が発生しているのも事実です。ますます困難を抱えて行く現状の中でどのように医療者は取り組んで行けばよいのでしょうか？例えば難病の芸術家がその身を削って生まれた芸術作品からは、むしろ生きている証し、命の力が伝わってくるのを感じます。困難に必死に立ち向かうところにこそ命が輝くといえるのでしょう。そしてその困難に取り組む姿勢にこそ収斂が生まれて来るのではないかと思います。
患者の死は医療者にとっては敗北そのものであり、患者の死後に医療者がその遺族の精神的ケアをなされることはほとんど稀であり、通常は宗教者に委ねられることになります。
昨年９月９日、当時医学部６年生だった息子、春光はバイク事故を起こしました。救命救急医の方には全力をもって最大の医療を尽くしていただきましたが、残念ながら永眠いたしました。
私達は悲しみの極限にありましたが、大事故にも拘わらず息子の顔がきれいで、しかもうっすらと笑みを浮かべていたこと、告別式には数え切れない程の友人や師の参列を頂き、父親の私が把握出来なかった息子の生き様を窺い知ることが出来たこと。遺体となった息子が自分の存在を様々な形で示してくれたことが私の心の救いとなりました。本当に息子の死は無意味ではなかった。寿命と言う言葉はことぶきの命と書きます。天から与えられたまさしくことぶきの尊い命だと思えたのです。そして永田勝太郎博士の会頭講演が私のスピリチュアルペインを癒してくれたことは言うまでもありません。
ラスタム・ロイ氏はPerson＝Bodyではなく、Person＝Body＋Mind＋Spirit であると述べています。
今後において個々人の生老病死に即した最善の全人的個別化医療、そして人格を尊重したケアこそが望まれますが、現実には例えばクリニックの一人の医師とスタッフにとってはその活動能力の範疇を超えていると言わざるをえません。
当学会は専門性や分野にとらわれず、広く「個々人の健康・個別医療」を目指す会員で成り立っています。今こそ、既に当学会誌創刊号の巻頭言に示されている基本理念（①~⑤）の成就にむけ、多種多様で多数の医療者の英知と霊性を結集すべきであり、当学会が中枢となり医療機関や所属の枠を超越して全人的医療のネットワークを広げ、以って一人の個人に最適なパーソナライズド・メディシンを提供できるシステムを構築することは、焦眉の急を要するものでありましょう。

I would like to refer to four fundamental difficulties; to start life, to age, to suffer from pains, and to die, which we cannot avoid as long as we are human-beings.
As an example in modern medication, life-prolonging treatment can be raised. Although severe premature infants are successfully given birth nowadays, in some cases, artificially ending the life of a newborn is forced to carry out for hopeless infants nurtured by life-prolonging treatments. It causes intolerable dilemma to the family and the health care providers.
Medical genetics might allow us to manage aging in future, and to enjoy an extremely long life. However, euphoria would not be fulfilled by having a long life. I strongly believe there is a “Meaning in Life,” in other words, “Quality of Life” is substantial. Gross National Happiness in the Kingdom of Bhutan could be taken as an example. Several kinds of distortions brought about by a super aging society should be acknowledged, understood, and then should be extracted.
Getting rid of all diseases is an everlasting challenge for us. Nevertheless, various new unknown incurable diseases have arisen endlessly despite of remarkable progress of medicine. This is the reality. How can we cope with such an unbearable reality then? Why are we sometimes inspired by the stronger power of life and splendid aura of serious patients? I definitely insist that it is because a life is brightened up when one faces up bravery to hardship.
The patient’s death itself may be a defeat for us and spiritual treatment would be barely made after the death. It is usually approached religiously. September 9th, 2011, I lost my son, Harumitsu, who was 6th grade of medical school when he was killed by a traffic accident. Emergency Life-saving Technicians devoted all possible efforts on him but he died; his face was not injured much unbelievably, and looked smiling even. So many of his friends and teachers came to his funeral, and I could get to know from them some new aspects of my son, which I hardly knew as his father. My practically non-existed son showed us his natural existence through their memories. They helped me a lot to find a way to get out of my deep depression from his loss. “Life time” in Japanese pronounced “Jumyo,” which means “gifted life” in the Chinese characters used for the word. This definition made me believe that my son has achieved his gifted life. On the other hand, Dr.Nagata’s lecture in his latest conference also saved my spiritual pain.
Dr.Rustum Roy indicates that “Person=Body+Mind+Spirit” instead of “Person=Body.” Respective comprehensive medications come along with four respective elements inevitable in our life; Birth, Aging, Sickness, and Death. However, in reality, it is impossible for the doctor and his co-medical staff in a clinic, to handle such expected medication.
This conference is supported by those, who are taking a challenge to a wide range of different medications and personalized medicine. Now, to reach the fundamental theme mentioned in the preface, the First Journal of the International Society of Integrative Medicine, we should compile various kinds of our knowledge as well as our experiences, and we should become the backbone to build the network for addressing comprehensive medicine. I believe this act will enable us to come up with the most appropriate personalized medication system in near future.
